By Dr. Jayadeva Ph.

One fine morning while taking a shower, Tombisana found a small lump in her breast. That night she kept thinking whether to be concern of the lump or ignore it. She is 32 years old, unmarried and otherwise a healthy and active person. Tombisana was always a bit shy and this prevented her to go to a doctor and consult about the breast lump. She eventually decided to ignore it.

3 years passed, the lump was painful and her skin over it was red and thick. After consulting a doctor and taking appropriate investigations, she got the shock when the lump turned out to be a cancer!

She thought for a while and regretted that she should have thrown her shyness away and done a check-up at the earliest when the lesion was pre-malignant.

What mistake did she do?

Breast cancer is the most common cause for death in women around the world.Global breast cancer incidence increased from 641,000 (95% confidence intervals 610,000 -750,000) cases in 1980 to 1,643,000 (1,421,000—1,782,000) cases in 2010, an annual increase of 3•1%. Over 100,000 new breast cancer patients are estimated to be diagnosed annually in India.

Previously, it affect women living in the western countries more often, probably due to their lifestyle and eating habits. Now, India is increasingly facing the problem of breast cancer. Same can be said for places like Manipur, where women may face breast diseases like any of her counterparts in the world.

Many newer investigations are present in the world to detect breast cancer. But the first test is always the self-examination. Women of any age can do a thorough self-examination of both the breast to check for any lump or swelling.

The most common mode of examination for detecting breast cancer is the mammography. Mammography is a type of specialised X-ray machine which can detect both benign and malignant lesions. Mammography is recommended for patient who are above the age group of 35 years but not recommended for patients who are pregnant, lactating or below 35 years due to radiation hazard.Mammographic positioning is also painful for the patient.

Even though PET and MRI scan has higher accuracy, its availability is a concern.

The easiest and the more comfortable type of investigations is ultrasound screening of breasts. Ultrasound is a specialised device that uses high frequency sound waves (inaudible to human and similar to sound waves used by bats!) to visualise the soft tissue of the body. It does not uses radiation like mammography. So, ultrasound can be used in any individual whether she is pregnant or younger individuals which are unfit for radiation exposure.

The sensitivity for detecting breast cancer ultrasound ranges from approximately 80 to 90 per cent, which hold very good for patients who are young or pregnant and even for those who cannot afford other higher imaging modalities like MRI and PET or patients who does not want to be exposed to unnecessary radiation for screening purpose by mammography.

Ultrasonography is indeed a simple and inexpensive tool for evaluation of breast lumps. Not only it provide prompt diagnosis but also it is safe and painless.

Ultrasound detects early stage cancers in women with mammographically negative dense breasts, with higher contribution in women younger than 35 years .It is useful in the evaluation of palpable masses not visible in mammographically dense breasts, abscesses and masses that are not completely evaluable with mammography and in young patients who are susceptible to radiation.

Fine needle examination of any breast lump done blindly may yield false negative cases, i.e. it may wrongly take the sample for the biopsy from the area which may be of benign nature, other than taking the sample from the exact malignant site. False negative results are seen in 4-10% and are most common in fibrotic or well differentiated tumours

With the help of the ultrasound guided fine needle aspiration, the aspiration of the malignant cells become more reliable. Thus, making the patient worry lesser!

New development in the field of ultrasonography like Elastography has been developed which follow the simple rule of elasticity , telling that the more the lump is hard, the more likely it is to be malignant. Other newer techniques includes contrast enhanced Sonography.

Nowadays, not only women are affected by breast cancer. Many cases of breast cancer in man has also been reported. But most of the cases goes unreported or gets reported very late in life as the usual false presumption that man does not have breast! That they have chest!!

Every men that I know have breasts. It may be a rudimentary gland for them, but they do have breast glands which can very much develop breast cancer at any stage of his life. Men should also be aware as much as their female counterparts. Actually, men should worry more as breast cancer is often more aggressive than women;the breast tissue in men are very close to the chest wall and it can metastasis (spread) faster than breast cancers of women.

Every year, October is taken as the month to create awareness for breast cancer. In US and many other countries, prominent personalities, breast cancer survivors and any individual who want to share their knowledge to tackle this dreaded disease.

Lastly, the mistake that our friend Tombisana did was nothing. She was simply unaware of the consequences of the simple breast lump. She was not aware that the simple breast lump may turn out malignant. She does not know what could be the next step for diagnosis and treatment. Now that we all know some few simple test to be followed if you find a breast lump. Never to ignore even the smallest of them!

The verdict of the high profile case against the murder of Dr Thingnam Kishan, Yumnam Token and Aribam Rajen has been given. Hopeson Ningshen, a self-styled lieutenant colonel of the National Socialist Council of Nagalim (Isac-Muivah) has been convicted of murder by the special court of the Central Bureau of Investigation at Patiala House Court, New Delhi. Hopeson has been awarded life imprisonment. It was in the year 2009; Dr Kishan was then serving as a Sub Divisional Officer at Kashom Khulen under Ukhrul District. Investigation report reveals that Dr Kishan along with his two staff Y Token and A Rajen were kidnapped by the cadres of NSCN-IM near the Hungdung Gate in the afternoon of February 13, 2009. Dr Kishan had a meeting with the then District Commissioner of Ukhrul, Pankaj Kumar Pal on the morning of the same day. As per the report, the cadres of the NSCN – IM followed the trio from the DC office till Hungdung, which indicates that the cadres had a prior knowledge about the meeting at DC office. While the family of Dr Kishan have been holding the view that there is high likelihood of the then DC’s involvement in the murder, the CBI however has ruled out that possibility. Dr Kishan’s father not contented with the finding particularly with the DC and his probable involvement in the heinous crime, has approached the law court, pleading that the DC should be equally punished if he is found guilty. For that, we shall wait for the law to take its course.

The lifeless bodies of the three were found on February 17 wee hours morning at Taphou Kuki village under the Lukhrabi Bridge of Senapati District. That was on the fourth day after the kidnapping. Three of them were bludgeon to death, beyond recognition with their hands tied behind their back. It is important to recall that Dr Kishan’s family was quick to point out the possible involvement of NSCN – IM cadres behind the killing. Dr Kishan when he was alive had shared to his family members that there was active presence of the cadres of NSCN – IM in Ukhrul District. That the outfit’s influences were evident in most of the governmental works taken up in that area. The then Joint Action Committee formed against the killing, taking cue from the family’s standpoint, since its formation had maintained that the NSCN – IM cadres were involved in the crime, and that they must be punished according to the law of the land. It is also important to note that the outfit had initially tried to wash off their hands from the murder. The JAC was firm with their stand, and moreover there was mounting pressure from the public with agitations on the streets seen everywhere. Both in the valley and the hills, the spread and the scale of agitation was perhaps unprecedented. The NSCN – IM had to finally accept the involvement of its cadre, Hopeson, who gave the execution order to three other cadres namely: Abo Shibo, Johnny and Ashang. According to CBI the first two are still at large and the later has been claimed to have expired. The JAC must be congratulated for their stand to take the fight till the end. Their pointed demand was for punishing the malefactor; neither ex-gratia money nor jobs to the kin of the victims were included in their demand. The victims’ families should also be appreciated for their faith on the collective wisdom of the JAC members – though nothing on earth can replace their loss, even after the verdict.

CM to discuss cabinet reshuffle with party high command

Imphal, July 1: Chief Minister Okram Ibobi has left Imphal for New Delhi to attend a meeting of the AICC Monitoring group chaired by AK Anthony and will consult Sonia

Imphal, July 1: Chief Minister Okram Ibobi has left Imphal for New Delhi to attend a meeting of the AICC Monitoring group chaired by AK Anthony and will consult Sonia Gandhi on the matter of reshuffling ministries.

A source told IFP that the Chief Minister was not accompanied by deputy Chief Minister Gaikhangam.

The Chief Minister has assured 27 MLAs that he will discuss the matter with AICC leader Sonia Gandhi.

They are awaiting response from the party high command on the mid-term reshuffle of 10 ministries barring the Chief Minister and Deputy Chief Minister, added the source.

On the other hand, a minister on the condition of anonymity has disclosed that even if the Chief Minister has assured a possible mid-term reshuffle, there will not be a complete reshuffle.

“Among the MLAs who are demanding for a reshuffle there are so many who had completed full terms as ministers. We didn’t cause any obstructions during their tenures as ministers,” he said.

The MLAs have demanded an assurance from Chief Minister for a reshuffle before the July 11 Assembly budget session. The MLAs have cited the establishment of the NDA government at the centre for the need to reshuffle the ministries as it would strengthen the Congress and bring confidence to the people about the party. They are of the opinion that the Congress is going to be in a different position in the next Assembly election.

Among the MLAs demanding the mid-term reshuffle there are many who have had successful tenures as ministers in the past.

The development works of their constituency are looked after by the current ministers. It hinders the developmental work in their local areas which has caused the lack of faith among the local public. This will lead to a downfall of the Congress party in the general elections in 2017, they had complained.

A source said that even among the 27 MLAs who met the Chief Minister on Monday, some of them were in the favour of the current ministers. They acknowledged that the ministers were doing well in every field and if they support the ministers in undertaking various development programmes, there will be good governance in the future.

Imphal, July 1: No community can develop if it’s disunited. Development awaits us if stay united and co-exist peacefully. This was stated by Saikul MLA Yamthong Haokip while inaugurating the 37th three-days-long youth-cum-sports meet organised by Salam Patong Sporting Youth Club under the theme ‘Harnessing Talent for Change’.

He further said that the Vaiphei community originated in Salam Patong village after leaving the then Burma, now known as Myanmar. The club was established in 1977 but till now there is no development in the area.

“We need unity among the Vaipheis if we have to stay together as one and for the Vaipheis to shine in future.”

He said that he had seen little unity here in Salam Palong village since the last few years.

Talking about his decision to repair the existing Salam Patong Public Health Sub-Centre, he said that he had sanctioned Rupees 25 lakhs from his local area development fund to repair the run-down building of Salam Patong PHC.

Kuki communities have the full-fledged power to exercise their rights to develop the village though there are youths, women and other village authorities or committee.

Assuring the people of Salam Patong that he will construct a bridge over the Khoikai River, he said that the road which connects to the rest of the state is in a very vulnerable condition.

“If there is disunity among the people, no development can be initiated in the area even after requests are made. If there is unity among the people, I will work in the interest of people after looking what is lacking in the area,” he added.

In the inauguration function, Assistant professor Thanthang Vaiphei of MU and SDO/SDM, Henglep Soiminlein Vaiphei were present as the president and guest of honour respectively. Colourful and cultural dance were performed by the youths of Vaiphei.

IMPHAL, July 1: The JACs formed in connection with the alleged killing of SI Krishnadas and the alleged detention of Krishnachandra by the police over the former’s killing case called a joint press conference today at the Manipur Press Club.

Spokesperson Th Joshikanta said that the police are unable to produce the culprits involved in the killing and is instead trying to hide the true findings of the case.

The agitation launched by the JAC is in demand of justice for victim’s family and there is no attempt from the JAC’s side to make it communal, he asserted.

To continue with the agitation, the JAC will start handing out leaflets on the incident and launched poster campaigns at different parts of the State from today onwards, he said.

Takhellambam Bebina who had already given her statement before the police said she was running the chicken centre jointly along with the family of the victim SI Krishnadas.

While she sells cooked meat, the SI’s family used to sell chicken meat, she said.

Recounting the incidents of the day, Bebina said that on the fateful day of June 21, she had open her shop around 10am, she said and continued that around 7pm Krishnadas had come to the shop and stayed in front of the shop for some time waiting for his wife.
